Form F - Application for Permit for Concentrated Animal Feeding Operation
This is the same form that was previously used for the construction permit
application. This form is the basic application that will need applicant
information such as contact information, general description of the proposed
operation and the specific location of the operation. Click here to download

Permit Fee: $5000 annually
If you are applying for a renewal, do not submit a permit fee; your permit fee
will be billed to you on an annual basis.

Construction Certification
For new facilities, submit engineering certification for construction completed
under the construction permit. Submit as-built engineering plans if
construction permit was not obtained.

Submit the following if any of the information is NOT already on file with
the construction application.
This information must be updated as part of the permit renewal application:
a. Narrative Summary of the Project Design
b. Location Map
c. General layout drawings
d. Operation and Maintenance Plan
e. Geologic Report by the Division of Geology and Land Survey
f. Soils Report
g. Design Worksheets
h. Construction Plan Drawing and Specifications
i. Nutrient Management Plan
j. Verification of compliance with Neighbor Notice

Name and address of any local planning and zoning authority
This provides MDNR with the contact information for local planning and zoning
authority to ensure proper notification and proper actions are taken.

Other relevant information, as needed
Other information may be requested to determine compliance with the Missouri
Clean Water Law or any regulations that are required by MDNR.

Mail List
Submit a current mailing list for all adjacent property owners and all property
owners of land application sites.
