Form F - Application for Permit for a Concentrated Animal Feeding Operation

contact information, general description of the proposed operation and the

Permit Fee
The fee associated with the construction permit application depends on the
design flow capacity of the proposed waste treatment of the operation. $750 is
the fee associated with less than 500,000 gallons per day design flow and $2,200
for 500,000 gallons per day or more design flow. Currently in the state of
Missouri, there are not any livestock waste facilities that have meet the
500,000 gallons per day threshold.

Name and address of any local planning and zoning authority
This provides MDNR with the contact information for local planning and zoning
authority to ensure proper notification and proper actions are taken.

Storm Water Land Disturbance Permits
Storm water permits are needed if the land disturbance associated with
construction will exceed one acre or more. Land disturbance can be defined as
clearing, grubbing, excavating, grading, and other activity that results in the
destruction of the root zone.
After determining the guidelines and requirements to fall under a general
permit, you can determine what forms need to be filed by measuring how much land
disturbance will be needed for the operation.
a. Less Than One Acre of Land Disturbance
Sites that disturb less than one acre of total land area that are not part of a
common plan or sales and that do not cause any violations of water quality
standards and are not otherwise designated by the department as requiring a
storm water permit.
b. One to Five Acres of Land Disturbance
Sites that disturb less than five acres but more than one acre are required to
submit:

Engineering Documents
Engineering documents that are to be submitted must be prepared by a registered
professional engineer licensed to practice in Missouri and will bear the imprint
of his/her seal and signature. Specific information that must be included with
the construction application includes:
a. Narrative summary of the project design
b. Location map A topographic map showing buildings, lagoons, land application
sites, property boundary and general features.
c. General layout drawings
d. Operation and Maintenance Plan
e. Design Worksheets
f. Construction Plan Drawings and Specifications

Geologic Report by the Division of Geology and Land Survey
A geologic report is required for each site where a lagoon or other earthen
basin is proposed for wastewater, contaminated stormwater or liquid manure. The
report will give a rating of the site which is based on the expected subsurface
variability and regional groundwater influences that would be expected. If a
severe rating is given for collapse potential, earthen basins will not be
allowed. The engineer and applicant should be aware that this report will take
between 30-45 days to receive the geologic evaluation. Click here to download

Soils Report
A soils investigation is required to determine whether the soils on-site are
suitable for pond seals. The soils investigation may be conducted by an
engineer, soil scientist or NRCS soil technician.

Nutrient Management Plan
The nutrient management plan means the use of either the conservative or plant
available nitrogen (PAN) approach for determining nitrogen application rates
concerning waste application.
a. The conservative management approach means the annual application rate is
based on applying 100 pounds per acre of total nitrogen. Total nitrogen is based
on total nitrogen content of the wastes at the time of application.
b. The PAN approach means the annual application rate is based on the available
nitrogen uptake of the plants grown using annual testing of process wastes and
soils to determine nitrogen requirements. Use the following links to download

Verification of compliance with Neighbor Notice
This to provide the proof of providing neighbor notification to all adjoining
property owners within 1 ½ times the buffer distance and county governing
bodies.

Other information may be requested to determine compliance with the Missouri
Clean Water Law or any regulations that are required by MDNR.
